This survey will focus on incidents of school violence, both averted and carried out. The survey is split up into four sections, which are as follows: School Information, Incident Information, Suspect Information, and Additional (optional) Information. The goal of this survey is to better understand incidents of school violence, specifically, what was done to avert an incident (if applicable), to improve school safety, and prevent further tragedies before they are carried out.

These responses are anonymous, unless you wish to provide your contact information (which is maintained confidentially), so that we may follow up, if needed, on incident information or your submission.

Fields with an * are required to ensure the integrity of the data. To ensure that accurate information is captured and used for resources and training to prevent acts of violence and keep our schools safe, we have a strict review and publication process that requires certain information. All other fields not marked are optional.
